Client Agreement & Acknowledgment
Before your session, please take a quick moment to review the following:
By booking and attending your appointment with Precious Peek 5D Elective Ultrasound Studio, you acknowledge and agree to the following:
1. Prenatal Care
I confirm that I am currently under the care of a licensed healthcare provider and will continue to receive regular prenatal care. I understand that this ultrasound is not a substitute for medical care.
2. Elective, Non-Diagnostic Session
I understand that all services provided are elective and performed for bonding and keepsake purposes only.
No medical evaluation, diagnosis, or assessment of fetal health will be performed. Any questions or concerns regarding my pregnancy will be directed to my healthcare provider.
3. No Medical Reliance
I understand that nothing seen, heard, or discussed during my session should be interpreted as medical information or advice.
4. No Guarantee of Results
Every baby is different, and they definitely run the show.
I understand that image quality, baby’s position, and the ability to capture certain views or determine gender cannot be guaranteed.
5. Image Quality & Session Limitations
I understand that image quality can be affected by factors outside of the studio’s control, including baby’s position, placenta placement, amniotic fluid levels, gestational age, and maternal anatomy.
Session time will not be extended due to lack of cooperation from baby.
6. Rescan Policy
If eligible based on package and gestational age, a complimentary rescan may be offered.
Rescans are not guaranteed, are subject to availability, and do not apply to all sessions or promotional packages.
7. Refund Policy
All services are non-refundable once rendered.
No refunds will be issued due to image quality, baby’s positioning, or inability to obtain desired images or gender confirmation.
8. Assumption of Risk & Liability Release
I understand that this is a voluntary, elective ultrasound.
I acknowledge that while ultrasound has been widely used and is considered safe when performed appropriately, all procedures carry some level of unknown risk.
I voluntarily assume all risks and agree to release, waive, and hold harmless Precious Peek 5D Elective Ultrasound Studio LLC, its owner, employees, contractors, and affiliates from any and all claims, liabilities, damages, or causes of action arising from or related to my ultrasound session.
This includes, but is not limited to, any claims related to image quality, fetal positioning, gender determination, or the inability to obtain desired images or confirm pregnancy.
I understand that this release applies to any and all damages or injuries, whether known or unknown, and regardless of when they may arise or be discovered.
9. Medical Concerns
If anything concerning is suspected during the session, I understand that I will be advised to contact my healthcare provider. Precious Peek 5D does not diagnose or communicate medical findings.
10. Cancellations & Rescheduling
We kindly ask for at least 48 hours notice to cancel or reschedule.
Late cancellations or no-shows may result in loss of your deposit. Rescheduling is subject to availability.
11. Photo & Video Release
I give permission for non-identifiable images or short clips from my session to be used for marketing.
No personal or identifying information will ever be shared. I understand I may opt out by notifying the studio prior to my appointment.
Brief phone use, short clips, and photos may be taken to capture special moments outside Precious Peek 5D. Full-session recording, extended filming, or capturing the sonographer’s screen or workspace without permission is prohibited.
12. Medical Records and Documentation
Precious Peek does not provide, store, or share medical records, reports, or interpretations for elective ultrasound sessions. These self-referred, non-diagnostic appointments are private and intended for bonding and keepsakes. Clients receive photos and videos for personal and entertainment purposes only. Requests for medical documentation from elective appointments will not be fulfilled.
13. Image & Digital File Responsibility
Once images, videos, or digital files have been delivered via email, text, or online gallery, they become the client’s responsibility. Precious Peek 5D is not responsible for lost, deleted, or misplaced files after delivery. While we may retain files for a limited time, we do not guarantee long-term storage or availability.
Clients are strongly encouraged to download, back up, and save all images and videos promptly upon receipt.
